New Year, New You!

2013 has been a busy year and we are looking forward to seeing you in 2014!

As we say goodbye to the current year, I wanted to thank my loyal patients for your support and wish you all a wonderful 2014. Our office building has seen a major construction project this past year (still unfinished), and we used the opportunity to perform a complete remodel of our office. What else happened in 2013?

Everything! We moved a step closer to the Holy Grail of plastic surgery.

This year saw the development of what may be the single most revolutionary procedure for rejuvenating aging skin and soft tissue. Not by pulling, freezing, burning or lasering, but truly repairing and restoring damaged skin. We call it Nano Stem Serum, and this is how it works:

I remain a leading edge surgeon and advocate for Natural Fat Transfer, where we use natural adipose (fat) tissue to fill lines and soften the thinning face as it ages. For reference, you can hear me talk about this in multiple short videos on my website. We now take the remaining, purified fat and gently process it to extract stem cells from the tissue and concentrate them into a smooth liquid. This serum is thin enough to be injected directly into damaged skin.

We can now make skin more healthy by placing growth factors and reparative stem cells exactly where we want them - in the skin! Injected into crows’ feet, upper lips lines and crepey skin, we see thickening and a smoother appearance. In the décolletage, we see softening and disappearance of wrinkles. We are also studying the effect of placing Nano Stem Serum into the lower eyelid skin for improvement of dark circles and skin discoloration.

Get a Sexier Silhouette with a Breast Lift

Your breasts change over time. Gravity, pregnancy and breast-feeding, even weight loss can all take their toll on the shape, tone, and volume of your breasts. A breast lift, also known as mastopexy, can restore the youthful contour of your breasts, giving you a perkier profile. The procedure raises and firms your breasts by repositioning the nipples, tightening the skin and reshaping the internal tissue. Some women choose to have breast augmentation at the same time, to add fullness. Others choose breast reduction in conjunction with a breast lift to achieve smaller firmer breasts.

Is a Breast Lift Right For You?

Mastopexy can be performed at any age after breasts are fully developed. Breasts that are large and heavy can be lifted, but the results may not be as long-lasting as when the procedure is done on smaller breasts.

Since the milk ducts and nipples are left intact, breast lift surgery usually will not affect your ability to breast-feed; however, pregnancy and breast-feeding may have significant and unpredictable effects on the size and shape of your breasts.

How is a Breast Lift Performed?

A breast lift procedure (mastopexy) raises and tightens your breasts. Nipples are elevated and the areolae are usually reduced in size.

You and Dr. Koplin will determine the specific method for your breast lift, taking into account your anatomy and desired results. The most common methods he employs use a two-part incision. One part of the incision is made around the areola (nipple area). The other part runs vertically from the bottom edge of the areola to the crease underneath the breast.

Sometimes a third component is necessary. This adds a horizontal incision beneath the breast, which follows the natural curve of the breast crease. Dr. Koplin will spend a lot of time discussing the options with you, to help you choose the best technique based upon the shape of your breast and desired results.

In the actual surgery, the nipples remain attached to underlying tissue as they are moved to their higher position – much like a button being moved to a higher buttonhole. This preserves sensation and, almost always, the ability to breast-feed.

The Breast Lift Consultation with Dr. Koplin -- Unique For You!

Dr. Koplin will personally meet with you so that he understands your expectations and desired outcomes for a breast lift. During this initial meeting, Dr. Koplin will examine your breasts and perhaps take photographs for your medical record. He will consider such factors as the size and shape of your breasts, the size of your areolae, and the extent of sagging so that he can advise you on the technique that will work best for you.

Dr. Koplin will ask you a number of questions about your health, desires, and lifestyle. He’ll need to know your medical history and the results of any mammograms or biopsies. It’s also important to tell Dr. Koplin if you plan to have children and/or breast-feed in the future, and if you plan to lose weight.

If you have opted for breast augmentation or reduction in addition to your breast lift, Dr. Koplin will explain the procedures and discuss your options.

Where is the Breast Lift Operation Performed?

Your custom procedure will be performed in a brand-new, state-of-the-art facility associated with our medical office. We are fully accredited by Medicare and AAAASF, and are held to the same standards as an operating room in a private or university hospital.

Safety and Comfort with Anesthesia

General anesthesia is the gold standard for this operation. Under this type of anesthesia, you are fully asleep; there is no pain and no awareness. Alternatively, a breast lift may be performed using local anesthesia and intravenous sedation. Safety and comfort are always paramount. Your anesthesia provider has a wealth of experience in surgical procedures, safety, and care.

Fully-Staffed Recovery Room

You will spend one to two hours in a fully-monitored and state-of-the art recovery room at the surgery center. Your registered nurse will be at your side every moment to attend to your safety and comfort.

Breast Lift Recovery and Convalescence

After your breast lift procedure, you will be placed in a soft surgical wrapping for comfort and support. Dr. Koplin almost never uses drains or tubes with this type of surgery. You should be walking under your own strength immediately after your breast lift surgery, and enjoying a normal dinner at home. It is very important that you walk a few minutes every few hours to reduce the risk of blood clot formation in your legs.

You have several options for recovery: you may choose to have professional care, to stay in a recovery center, or recover at home. If you wish to recover at home, you must have someone stay with you the first night. We’ll send you home with specific care instructions, as well as information about medications that can aid healing and reduce the potential for infection, specific concerns to look for at the surgical site or in your overall health, and when to follow-up with Dr. Koplin.

What to Expect After a Breast Lift:

The First Week:

The results of your breast lift surgery are immediately visible. Your breasts will be firmer and more uplifted. Your nipples will no longer droop, and the size of your areolae will be aesthetically pleasing.

A day or two after surgery, you should be up and about. While it will take several days to return to more normal activities after your breast lift, it is important to your recovery that you be out of bed and relatively active.

You may need to sleep on your back to avoid pressure on your breasts.

We’ll likely see you the morning following your surgery, at which time we’ll remove your supportive bandages and place you in a soft comfortable bra (which we provide).

Most of our patients are showering the second day after surgery!

Dr. Koplin exclusively uses dissolving sutures that are hidden under the skin, so there are none to be removed and never any “Frankenstein” marks!

Some discoloration and swelling will occur initially, but this will disappear quickly, with any residual swelling resolving within a month or so. The first few days following your breast lift, you may feel stiff and sore, although many patients remark that they have absolutely no pain after surgery. Your breasts may feel tight and sensitive to the touch, and your skin may feel warm or itchy. You should not lift, push or pull anything, or engage in any strenuous activity or twisting of the upper body.

The Second Week and Beyond:

You’ll be able to return to work in a week or two, depending on the level of physical activity required by your job.

You’ll also be able to resume most of your typical activities, including some form of mild exercise, after several weeks.

You may continue to experience some mild, periodic discomfort during this time, but such feelings are normal. Severe pain should be reported to Dr. Koplin.

The appearance of your breasts will change. It typically takes three months or longer before you achieve your final look and feel to your breasts.

Incisions will initially be red or pink in color. They’ll heal and fade over time. Dr. Koplin carefully conceals your incisions in areas easily covered by clothing, even low-cut necklines. We will spend quality time with you after surgery to carefully monitor the progress of your healing and interact in whatever appropriate manner is necessary. This may include scar creams (such as BioCornium®), silicone gel strips, tapes containing medications and laser therapy.
