New Year, New You!

2013 has been a busy year and we are looking forward to seeing you in 2014!

As we say goodbye to the current year, I wanted to thank my loyal patients for your support and wish you all a wonderful 2014. Our office building has seen a major construction project this past year (still unfinished), and we used the opportunity to perform a complete remodel of our office. What else happened in 2013?

Everything! We moved a step closer to the Holy Grail of plastic surgery.

This year saw the development of what may be the single most revolutionary procedure for rejuvenating aging skin and soft tissue. Not by pulling, freezing, burning or lasering, but truly repairing and restoring damaged skin. We call it Nano Stem Serum, and this is how it works:

I remain a leading edge surgeon and advocate for Natural Fat Transfer, where we use natural adipose (fat) tissue to fill lines and soften the thinning face as it ages. For reference, you can hear me talk about this in multiple short videos on my website. We now take the remaining, purified fat and gently process it to extract stem cells from the tissue and concentrate them into a smooth liquid. This serum is thin enough to be injected directly into damaged skin.

We can now make skin more healthy by placing growth factors and reparative stem cells exactly where we want them - in the skin! Injected into crows’ feet, upper lips lines and crepey skin, we see thickening and a smoother appearance. In the décolletage, we see softening and disappearance of wrinkles. We are also studying the effect of placing Nano Stem Serum into the lower eyelid skin for improvement of dark circles and skin discoloration.

Sculpt your Body with Liposuction

Liposuction may be the answer if you can’t diet or exercise enough to get the trim figure you’d like. If you have stubborn areas of excess fat, liposuction, also known as liposculpture or body contouring, removes deposits of excess fat from specific areas of your body, face, or neck. Liposuction can give you a flatter abdomen, slimmer hips and thighs, and shapelier calves and ankles. It can even eliminate a double chin.

Is Liposuction Right For You?

Liposuction is most commonly recommended for men and women of normal weight who simply want to improve their body contour. If you have deposits of excess fat that make certain body areas appear disproportionately large, this surgical body contouring procedure can produce excellent results. Although liposuction is not a treatment for obesity, it can permanently alter body shape and is the only way to eliminate fat deposits that do not respond to diet and exercise.

People of almost any age may undergo liposuction; however, patients with good skin elasticity will achieve the smoothest contour after fat is removed. You may be a good candidate for liposuction if you desire fat removal or contouring in any of the following areas:

It's important to be clear about a few "cans and can'ts" related to liposuction.

Liposuction can be performed on several areas of your body at the same time, or be done in conjunction with other cosmetic surgery procedures. Dr. Koplin will carefully review with you what he feels is a smart and safe combination and create the “perfect” plan.

It can also be used to treat some cases of gynecomastia, or male breast enlargement, which occurs in both teenagers and adult men.

How is Liposuction Performed?

To perform liposuction, one or more tiny incisions are made near the area to be treated. Whenever possible, incisions are placed within the natural folds or contour lines of the skin so that they are inconspicuous. Dr. Koplin then places a slim, hollow tube called a cannula through the incision, so that its tip penetrates the underlying fat. After the cannnula is connected by flexible tubing to a suction source, Dr. Koplin moves the cannula back and forth gently through the fat, which is vacuumed into the tube. Several variations to the basic liposuction technique have been introduced.

Tighter Skin, Smoother Shaping and Faster Healing

“PHAST” Lipo is a technique Dr. Koplin uses which optimally prepares the fat for removal and sculpts the treated areas to assure maximal smoothness and skin tightening. Dr. Koplin has been using and perfecting this technique for many years, and finds it better, safer and much more controllable than “laser” procedures such as Smart or Slim Lipo. Instead of “melting” fat with laser or ultrasound before removal (which can leave dead fat cells following liposuction), the fat is softened and loosened by using a special cannula, similar to shaking a tree to get ripe fruit off before harvest. Following fat removal using a thin, delicate cannula, the sculpting is continued by using the special cannula to allow the remaining fat to be massaged and molded into a smooth protective healing layer. Dr. Koplin calls this “auto-liposhift”, where the small, free fat particles can find their way into areas where they can nourish and soften the healing process. The auto-liposhift also allows the skin to tighten and reshape itself to the new underlying contour.

Dr. Koplin will personally meet with you so that he understands your motives and expectations for liposuction. He’ll examine you, discuss your options, and offer recommendations. During this initial meeting, Dr. Koplin will ask you a number of questions about your health, desires and lifestyle. He’ll also discuss likely outcomes of liposuction and any risks or potential complications. Special considerations are needed when large amounts—usually more than 5 liters— of fat are suctioned, and Dr. Koplin may recommend secondary procedures to tighten excess skin.

Your custom procedure will be performed in a brand-new, state-of-the-art facility associated with our medical office. We are fully accredited by Medicare and AAAASF, and are held to the same standards as an operating room in a private or university hospital.

Safety and Comfort with Anesthesia

Medications are administered for your comfort during liposuction. Choices include intravenous sedation or general anesthesia: Dr. Koplin will recommend the best anesthesia for you. Your anesthesia provider has a wealth of experience. Your safety and comfort are always paramount.

Fully Staffed Recovery Room

You will spend one to two hours in a fully-monitored and state-of-the art recovery room at the surgery center. Your registered nurse will be at your side every moment to attend to your safety and comfort.

Liposuction Recovery and Convalescence

You have several options: you may choose to have professional care, to stay in a recovery center, or recover at home. If you wish to recover at home, you must have someone stay with you the first night. We will send you home with precise instructions and a "care package" of supplies and medications.

We will provide a custom-fitting compression garment to mould and shape the skin to your new body contours.


The First Week:

During the first 48 hours following surgery, you’ll experience some degree of swelling and bruising. Rest quietly the first few days after surgery, but we encourage you to walk around and be out of bed.

Do not take aspirin or certain anti-inflammatory medications.

The Second Week and Beyond:

Bruising typically disappears between seven and fourteen days. You will still have some swelling, which can take three months to completely resolve as we actively encourage our patients to return to normal activities quickly. Most of our patients wear their custom garments a minimum of two weeks, when they can then resume active exercise.
